Likely causes

- Carefully lift the cable back into the center groove of the pulley wheel by hand.
- Slowly move the cable through its full range by hand (with no weight selected) to confirm it stays seated.
- Reinsert the weight pin and test with a very light weight first, watching the cable's path.
- If the cable keeps jumping off the same pulley, inspect the groove for wear or the bracket for bending.
- Avoid dropping weight quickly or using jerky form, which is the most common cause of this happening again.
